MEDICAL SOCIAL WORK:

• Consulting service to help

patients and their families

access community resources

and social services

· A patient will be considered to be homebound if

he/she has a condition due to an illness or injury

that restricts his/her ability to leave his/her place

of residence except with the aid of supportive

devices such as crutches, canes, wheelchairs and

walkers, the use of special transportation, or the

assistance of another person or if leaving home is

medically contraindicated.

· Any patient under the care of a physician who has

a need for skilled nursing care, physical therapy,

occupational therapy or speech therapy services.

Hours of Operation:

Monday through Friday 9:00 a.m. to 5:00 p.m.

We have a nurse on-call 24 hours a day, 7 days a week
